2024-2025 Application and Other Forms
Enrollment for 2024-2025 is open! Applications can be electronically submitted via email or printed out. Once completed, please email to admissions@villageschooloflouisville.org or mail to: Village School of Louisville, Attn: Selection Committee, 770 Eastern Parkway, Louisville, KY 40217.
In addition to submitting applications, please submit a $20, non-refundable application fee per family regardless of how many students you are applying for. We accept either a check mailed to the above address or online payments to our Venmo account: @villageschooloflouisville. Please contact us if paying the $20 fee presents a significant burden.

Application for Sliding Scale Placement
Applications for sliding scale funds are open for the 2024-2025 academic year. Please use the application above to apply and be sure to submit all required documentation along with the application.
Sliding Scale applications for the 2025-2026 year will open to returning families on September 27, 2024 (due November 8) and new families on October 18, 2024 (due December 20).
